ID Cards Overview
Identification cards must be presented each time an insured receives medical, dental, vision, or pharmacy services. Medical/Rx ID CardsWellmark provides each Member with medical coverage an ID card that shows the account number of your insurance program, the Member's name and the Member's identification number. There is a toll-free number printed on the card that will enable the provider to verify benefits. New Members will automatically receive a card when they are enrolled. Members with single coverage receive one ID card and Members with family coverage receive two ID cards. Please Limited Benefit Medical Plan ID CardsHM Life prepares ID cards for all Members covered under the plan. ID cards are mailed within 15 days after receiving enrollment information. Members should present their ID cards each time they visit a provider. To request replacement or additional ID cards, Members may call HM Life at (800)481-9979. Dental ID CardsDelta Dental prepares ID cards for all Members covered under the plan. ID cards are mailed within 15 days after receiving enrollment information. Members should present their ID cards each time they visit the dentist. To request replacement or additional ID cards, Members may call Delta Dental at 1-800-544-0718. Vision ID CardsAvesis prepares ID cards for all Members covered under the plan. ID cards are mailed within 15 days after receiving enrollment information. Members should present their ID cards each time they visit the vision care provider. To request replacement or additional ID cards, Members may call Avesis at (800)828-9341 or print one off their website at www.avesis.com.
